Substance Abuse and Mental Health Services Admin

Patient Health Questionnaire (PHQ) Screeners
Offers clinicians concise, self-administered screening and diagnostic tools for mental health disorders, which have been field-tested in office practice. Includes access to PHQ-9 and GAD-7 in addition to instructional manuals and additional screening tools.
